Less than 18 months after taking over exclusive ambulance services in Sonoma County\u2019s urban core, the local Fire District has begun negotiations to raise its rates by nearly 25%. The proposed increase would be implemented in two phases: an initial 9.9% hike requiring approval from county health authorities, and a second 14.35% increase subject to a financial audit and the Board of Supervisors\u2019 approval.<\/p>\n\n\n\n

According to Fire Chief Ron Busch, the request is driven by rising labor costs, an unfavorable shift in the insurance payor mix (with a higher proportion of Medicare and Medicaid patients), and a $700,000 increase in dispatch service fees. If approved, the basic life support ambulance transport would rise from $2,800 to $3,500, while advanced life support would jump from $3,100 to $3,900.<\/p>\n\n\n\n

The contract, which began in January 2024, is valued at $30 million annually over five years. It grants exclusive service rights to the district, in partnership with Medic Ambulance, for areas including Santa Rosa, Rohnert Park, and Sebastopol. For three decades, the service was managed by Sonoma Life Support (AMR), whose unsuccessful 2023 bid had proposed a flat rate of $3,900 per transport.<\/p>\n\n\n\n

The proposed hike also comes amid ongoing tensions between the district and county officials over how ambulance response times are calculated\u2014an essential metric in performance evaluations.
